Mail the Transcript Request Form to:

The following information is needed to process Transcript requests:

  • Full name and any prior names
  • Student identification number and/or Social Security Number
  • Birth date
  • Approximate dates of enrollment
  • Address where transcripts should be mailed
  • Student signature
  • Payment information

Please note:

  • The Transcript Division accepts Visa, Master Card, American Express, and Discover. Please list credit card number and expiration date.
  • No transcript will be issued until all financial obligations to the University have been cleared.
