Mail the Transcript Request Form to:

The following information is needed to process Transcript requests:

  • Full name and any prior names
  • Student identification number and/or Social Security Number
  • Birth date
  • Phone number or email address
  • Approximate dates of enrollment
  • Address where transcripts should be mailed
  • Student signature
  • Payment information

Please note:

  • The Transcript Division accepts Visa, Master Card, American Express, and Discover. Please present credit card number and expiration date. Cash, check or money order payable to the University of Utah is also accepted.
  • No transcript will be issued until all financial obligations to the University have been cleared.
