Submit a completed name change form, including the required documentation, to the Registrar's Office:

• In person to the Student Services Building (SSB), window 13 (second floor)
• By mail to: Office of the Registrar

Note:  If you are a student and also an employee of the University, contact the payroll reporter in your office. Once Human Resources changes your name it will carry through to your student record.
