VA CHAPTER 35 TRANSFER STUDENTS
If you have previously received benefits at another institution, there are a few forms we require from you in order to transfer your benefits to the University of Utah.
- A Change of Program or Place of Training Survivors' and Dependents' Education Assistance (22-5495) form.
- A Request for Benefits.
- If you will be over 26 years old before all the benefits are used, and the veteran was disabled or deceased after you turned 18 years old, please submit a copy of the veteran’s Disability Rating Letter or Death Certificate.
