VA CHAPTER 30 TRANSFER STUDENTS

If you have previously received benefits at another institution, there are a few forms we require from you in order to transfer your benefits to the University of Utah.

• A Change of Program or Place of Training (22-1995) form.
• A Request for Benefits form.
• A Military Service Credit application.
• The most recent copy of your DD-214 (Military Discharge Orders), or an LES (Leave & Earnings Statement) if you are still on active duty.
