VA CHAPTER 30 BENEFITS

First-time applicants need to submit the following:

• A 1990 Application for VA Education Benefits form.  You can also submit this form online through the VONAPP system.  If you choose to submit your application via the Internet, please notify us as soon as possible.
• A Request for Benefits.
• A Military Service Credit Request.
• The most recent copy of your DD-214 (Military Discharge Orders), or an LES (Leave & Earnings Statement) if you are still on active duty.  
• Please review our Information Sheet for additional instructions regarding your benefits at the University of Utah.
