VA CHAPTER 1606 TRANSFER STUDENTS

If you have previously received benefits at another institution, there are a few forms we require from you in order to transfer your benefits to the University of Utah.

• A Change of Program or Place of Training (22-1995) form.
• A Request for Benefits.
• A Military Service Credit Request.
• Also, please review our Information Sheet for additional instructions regarding your benefits at the University of Utah.

**Please note: If you are a current Chapter 1606 veteran, you may be eligible to apply for Chapter 1607.
